EARLY BIRD REGISTRATION DEADLINE: December 6, 2019

Important: do not attempt to complete fillable forms within your internet browser. Save the PDF forms to your computer and return via email. Signature pages may be digitally signed, photographed or scanned.

A. Conference Syllabus (one per presentation). We will be preparing a syllabus and must receive content materials for your presentation. Please provide either a short (2-4 page summary) of your session – including references, or your presentation slides. Please submit your materials by December 10, 2019, in Word or PowerPoint format only, we are not able to accept PDF submissions. See here for syllabus submission guidelines.

B. Registration (one per person). All speakers MUST register. Please register by December 6, 2019. See your speaker email for your registration code.

C. AV (Audio-Visual) Requirements (one per presentation). Please verify and confirm if you need additional equipment to what is already provided (eg. flip chart with markers, WiFi connection, etc) by submitting the AV form by December 10, 2019.

D. Disclosure of Conflict of Interest (one per person). Speakers must fully disclose any affiliation (financial or otherwise) with a commercial or other industry interest. Please complete the "Disclosure of Conflict of Interest" form and return it to our office by December 10, 2019.

E. Filming Consent Form (one per person). Please submit your filming consent form to Kathryn.

F. Moderator Introduction (50 words max) (one per person). The moderator for your session will be given the biography that was originally submitted. If you wish to update this information, please use the form and submit to Sarah by December 10, 2019.

E. Accommodation. We are pleased to offer a group rate of $155.00 + applicable taxes for a standard guest room (single/double). Reserve today to avoid disappointment and specify that you are booking under the UBC Interprofessional Continuing Education FASD 2019 Conference room block to receive the special rate.

Tip: If you are speaking in multiple types of presentations, please take careful note of the parts in blue so you do not submit redundant information.

APPLICABLE TO ALL SPEAKERS:

Please include 25% of your time for discussion/throughout the presentation. Please be ready with discussion questions to pose to the group to help facilitate discussion if needed. This is required for the conference in order to meet accreditation standards.
